The form will request a payment method to keep on file. Nothing will be charged until we talk to you. There is a $100 deposit for the initial consultation appointment, which will be applied to your bill at your first visit. If you cancel within 48 hours of the appointment, we will refund $75 of the deposit. We’ll send you a confirmation email once we’ve processed the intake on our end!
